Docetaxel
Indications/Uses
Breast cancer, non-small cell lung cancer, gastric cancer, head & neck cancer. Patients w/ prostate cancer who have undergone surgical or medical castration & in whom progression or recurrence has been observed.
Dosage/Direction for Use
Adult Breast cancer, non-small cell lung cancer, gastric cancer, head & neck cancer 60 mg/m2 IV infused over 1 hr or longer once daily at 3-4 wk interval. Max daily dose: 75 mg/m2. Prostate cancer 75 mg/m2 IV infused over 1 hr or longer once daily at a 3 wk interval.
Contraindications
History of serious hypersensitivity to docetaxel or prep containing polysorbate 80. Serious bone-marrow suppression; concomitant infection; fever suspected infection. Pregnancy.
Special Precautions
Careful administration in patients w/ bone marrow suppression; interstitial pneumonia or pulmonary fibrosis; liver or renal disorder; edema; patients who may become pregnant. Risk of serious bone marrow suppression. Perform frequent laboratory testing (eg, blood test). Observe for onset of infection & check the absence or presence of symptoms eg, neutropenia, elevated CRP, & fever. Consider other treatments regarding focus of brain metastasis. Serious symptoms of irritation. Perform adequate observation of cardiocirculatory system. Occurrence of acute leukemia & myelodysplastic syndrome w/ other antineoplastics or radiation therapy. Risk of testicular toxicity. Confirm that the patient is not pregnant before starting administration. Discontinue breast-feeding when administrating to nursing women. Elderly. Paed use.
Adverse Reactions
Bone-marrow suppression; shock symptom, anaphylaxis; jaundice, hepatic failure, hepatic function disorder; acute renal failure; interstitial pneumonia, pulmonary fibrosis; cardiac failure; disseminated intravascular coagulation (DIC), intestinal perforation, GI hemorrhage, ischemic colitis, colitis; ileus; acute resp distress syndrome; acute pancreatitis; Stevens-Johnson syndrome, toxic epidermal necrolysis (Lyell's syndrome), erythema multiforme; cardiac tamponade, pulmonary edema, edema & fluid retention; cardiac infarction, venous thromboembolism; infection, syndrome of inappropriate antidiuretic hormone secretion (SIADH); serious mucositis [eg, stomatitis, vasculitis, peripheral nerve disorder, peripheral motor disorder (eg, feeling of weakness in 4 extremities, & radiation recall phenomenon)].
Drug Interactions
Enhanced bone marrow suppression w/ other antineoplastics; radiation therapy. Radiation pneumonitis w/ radiation therapy. Increased blood conc w/ azole antifungals (eg, miconazole), erythromycin, clarithromycin, cyclosporine, & midazolam (CYP3A4 inhibitors).
